Web Portal (Medical Record)

  • The Web Portal is where you will find access to your secure on-campus medical record and is used for completing required medical forms, viewing and printing copies of your billing statements and immunizations and messaging the health center staff.

Student Statement of Understanding (SOU)

Commonwealth University (CU) continues to collaborate with the Pennsylvania Department of Health (PA DOH) to monitor any new public health risk or changes in public health guidance. This Student Statement of Understanding (SOU) puts forth the requirements that provide the most successful pathway to assure the safety of the University community.

After the skin test is placed, you must return to the office in 2-3 days for the final reading and to receive the paperwork test results. If you don’t return, you’ll need to be retested.

  • If a student is ill and unable to attend class, the student should read and follow the Course Syllabus regarding class absence policy.
  • If a professor or employer requires documentation for an illness or injury absence, the student should call the Campus Clinic on the same day of the missed class/work. We will provide further direction.
  • After an evaluation with Campus Clinic medical staff, the student may request a “Letter" for class/work absence which will be sent via their secure web portal to download and email.
  • If a student is evaluated by a medical provider outside of the Campus Clinic (ER, Urgent Care, PCP, Specialist, etc.), they should ask for a “doctor’s note” to use as proof of absence from class/work.

Extended Medical Illness (EMI)

  • An EMI is a formal notice sent over CU Succeed by the Campus Clinic staff if three (3) or more consecutive days of missed classes are due to an illness or injury.
  • Contact the Campus Clinic for more information.

The Campus Clinic will not provide any information regarding a student's medical situation to anyone: family, professors, friends, or even other health care facilities, without the student's written or verbal consent. Professors should not call the Campus Clinic to obtain any information about a medical absence. Students can refer to the CU Notice of Privacy Practices document in their Web Portal for more information.

Home Medication Management

  • The Mansfield Campus Clinic will not store, administer personal medications nor accept mailed packages or medications that belong to students.

Mailing medications to campus

  • Use Fed Ex, UPS, USPS, DHL, etc.
  • Address package as follows:

Your assigned building mailing address and Mailbox # (DO NOT USE YOUR ROOM #)

  • If signature is required, Mail Services will sign for it
  • Students will be notified via email from mailservices@mansfield.edu to retrieve a package from mail services. Packages are not available for pick-up until this email notification is sent.
  • Packages not picked up within 14 days of receiving the email notification will be returned to sender.

Special Note

  • The Mail Room are closed on weekends and holidays.
  • Refrigeration services are available until pick-up.

Storage of Refrigerated Medications (i.e. injectables, insulin)

Condoms

FREE condoms are available at the campus clinic office. Stop in and pick-up! Also available in the Pride Center, Women’s Center and Title IX office.

Depo-Provera

Depo Injections are administered at the Campus Clinic, but are not prescribed. Please call for more information.

Birth Control Pill

  • Prescription birth control pills are prescribed
  • If you are interested in discussing other birth control options, please contact your PCP, Laurel Health Center

Emergency Contraception

  • Emergency Contraception (EC) is available at the Campus Clinic. Please call for an appointment and for more information.
  • Emergency Contraception (EC) is available over-the-counter at the following locations near campus: CVS & Walmart or visit the Plan B One-Step website.
EC guidelines
  • EC is most effective if taken within 72 hours (3 days) of unprotected intercourse, but it may be effective up to 120 hours.
  • Call the Campus Clinic to schedule an appointment.
  • Prior to receiving emergency contraception at the Campus Clinic, a urine pregnancy test will be performed. Please be able to provide a urine specimen.
    • Birth control options and prevention of Sexually Transmitted Diseases (STD's) will be discussed.

STD Testing Clinics:

SelectPlan for Women

  • Application required.
  • Services include: health exams, routine health screenings and birth control methods. Education, family planning counseling and referrals to other health care providers and social services.
  • Prior to any care rendered in the SHS, the student’s home Medical Provider or Specialist must complete and sign the Medication, Suture Removal, and Wound Care Order Form.
  • The student can drop-off, send through Web Portal or fax the completed and signed form to the SHS.
  • Prior to receiving care in the SHS, the form must be reviewed and approved by a SHS Medical Provider.
  • If approved, the student will be notified to schedule an appointment.
  • If denied, the student will be notified to discuss other care options.

Medication Orders

Student must submit the order form at least one week prior to the anticipated administration of the medication.

Student must bring the medication to the SHS appointment. You may ask your prescribing physician to call the prescription to a Local Pharmacy.

Suture/Staple Removal

If sutures were placed in an Emergency Department (ED), the Medical Director will review the discharge instructions. 

Student must submit the ED discharge instructions or order form at least 4 days prior to the anticipated need for suture removal.

Wound Care

Student must submit the ED discharge instructions or order form at least one week prior to the anticipated need for wound care.

At any time during the treatment, the provider may request the student return to their outside provider for re-evaluation of care.

Need a copy of your medical records or medical information sent to another facility? 

Please read the following information carefully:

  • Per PA State Law, medical records are kept for a minimum of seven (7) years from the last encounter date before they are destroyed.
  • Current students may view and print a copy of your immunization record via — Health Center Web Portal.
  • In order to release your medical records, you must complete the Medical Records Release Form (* This form is not intended to be used as a blanket release form).
