Forms and Resources

  • Club Registration/ Officer Update Form
    Complete this form to make sure your club officers and their contact information is up to date!
  • Club Liability Waiver
    Every student who is a member of a club must complete a liability wavier for every club they are a member of. These forms act as a club roster.
  • Club Budget and Special Account Vouchers
    This form is used to request cash advances (up to $100), reimbursements, and purchase orders from club budget accounts and special accounts.
  • Account Balance Request Form
    Complete this form to request account balances for your club's budget and/or special account.
  • Travel Form
    If your group is traveling for a club activity you must complete a travel form. This form must be completed and returned to the Student Activities Office TWO WEEKS prior to the departure date of the trip.
  • Monthly Activity Report
    This form must be completed monthly for each club. It is an important assessment tool and tracks your club activity for the year.
  • Contract Request
    If your club is working with a vendor outside of the university and they don't provide you with a contract you can request one through the LHU SAS Inc. Information provided on this form will be used to create a contract! This request must be completed and submitted at least two weeks prior to the event.
  • Transfer of Funds Form
    This form is used for clubs to request their budget funds be moved into other areas of the budget. This form is reviewed and approved on a case by case basis.
  • Contingency Request
    Clubs should submit a contingency request form if they are looking for additional funding to support an existing item in their budget, or for new programming needs. This form is reviewed and approved on a case by case basis.
  • Fundraising Form
    All fundraisers must be approved prior to starting. Forms should be submitted at least two weeks in advance.
  • Event Registration Form
    Club events should be submitted for tracking by the Student Involvement Office and advertisement of events in the Student Involvement emails.
  • Conference Funding 
    LHU SAS, Inc. Conference funding is available for up to four (4) students. Additional Students may attend but must be funded through the club's special account. All funding requests must be submitted two (2) weeks prior to the conference travel.

The Lock Haven campus recognizes four national sororities and three national fraternities. During your time on campus, the fraternity and sorority life experience can provide valuable engagement opportunity for students.

The Community Service Office at Lock Haven offers students the opportunity to learn while serving others. We provide numerous local community service projects and partner with non-profits agencies to match your interests and expose you to hands on learning experiences.
